Our office is striving to keep our patients and staff healthy with the ever changing health situation. We are following infection control guidelines and following the CDC recommendations regarding the Coronavirus (COVID 19).
If you answer YES to any of the following questions, please contact our office as soon as possible and we will reschedule your appointment as needed to another date, to a phone appointment or possibly a telemedicine appointment.
1. Have you traveled in the last 14 days or do you live with someone that has traveled out of the country or to/from any city with an ongoing outbreak?
2. Do you personally feel sick or have you been exposed to or living with someone who is sick -- with fever, cough, sore throat or other flu like symptoms or upper respiratory symptoms?
3 Have been advised to self-quarantine or been advised to quarantine by health officials?
Should you wish to change any upcoming appointment to video or phone, please contact the office. Please limit the number of people coming with you to your appointment.
NEW Patients: Please contact us if you have symptoms listed above or have questions regarding traveling to the office. We will review each situation on a case by case basis and determine next steps for your particular situation.
